Referrals can be made using the referral form below, or the standard template of your EMR system. Please ensure that a patient email and mobile phone number is provided on the referral form.
For patients:
If you would like to see on of our physicians, please contact your primary care provider for a referral. Patients will be notified of appointment times by email or phone.
